Job summary

Garden Place Healthcare in Attleboro, MA is seeking a Licensed Social Worker (BSW) to lead resident care planning and discharge coordination in long-term care.

You will collaborate with families and the care team, provide counseling, assist with applications for assistance, and ensure adherence to regulations while supporting residents through transitions and changes in living arrangements.

Qualifications

• 1–3 years minimum long-term care experience.
• Bachelor's Degree in Social Work, Psychology or related field required.
• Licensed Social Worker (LSW/LSW-C) in the appropriate state or eligible for licensure required.

Responsibilities

• Promote positive morale and lead by example for residents and families.
• Coordinate admission planning, goals, and discharge planning with the care team.
• Perform screenings, plan of care, and necessary paperwork including MDS.
• Explain Resident Rights to new admissions and ensure admission agreements.
• Collaborate with families, Business Office and Medicare/Medicaid for financial assistance applications.
• Schedule interdisciplinary team meetings and write summaries; ensure follow-through.
• Provide guidance and counseling to residents and families on grief, coping, and changes in living arrangements.
• Coordinate discharge services and arrange follow-up care.
