Qualifications

• Bachelor's degree in Social Work or related field.
• Current CSW certificate.
• At least 1 year of interdisciplinary team experience, preferably in long-term care or hospice.
• Strong communication, critical thinking, and documentation skills.

• Proficiency with computer systems and maintaining confidentiality.
• Ability to collaborate effectively with residents, families, and care teams.

Responsibilities

• Conduct psychosocial assessments and provide counseling to residents and families.
• Support admissions, transitions, and adjustment to aging or illness.
• Work with the interdisciplinary team on care planning and documentation, including MDS.
• Assist with discharge planning and coordination of community resources.
• Advocate for residents and provide crisis intervention as needed.
• Maintain accurate documentation and support financial and insurance processes.
• Serve as a liaison with families, agencies, and mental health services.
