CHI Health Immanuel in Omaha seeks a Social Worker to support patients and families as they navigate complex healthcare needs, addressing psychosocial, financial, and discharge planning barriers to promote positive outcomes.

You will collaborate with physicians, nurses, and the care team to develop individualized plans, advocate for resources, and ensure smooth transitions of care with accurate documentation.

Qualifications

• Certified Social Worker (CSW) in Nebraska.
• Bachelor's degree in Social Work with intention to enter MSW.
• Basic Life Support (BLS) from the American Heart Association.

Responsibilities

• Assess patients' psychosocial, environmental, financial, and discharge planning needs to develop individualized plans of care.
• Collaborate with the interdisciplinary care team to coordinate safe, effective discharge plans and facilitate timely referrals to post-acute and community resources.
• Advocate for patients and families by connecting them with support services and assisting with applications for community and financial resources.
• Educate patients, families, and healthcare team members on available services and care options.
• Document assessments, interventions, and care coordination activities while maintaining compliance with standards.

By addressing social, emotional, financial, and discharge planning needs, you'll help remove barriers to care and support positive patient outcomes throughout the care journey.

Every day you will collaborate with patients, families, physicians, nurses, and the interdisciplinary care team to develop individualized plans of care and coordinate safe, timely transitions. You'll serve as a trusted advocate, connecting patients with community resources and support services that promote continued health and well-being beyond the hospital.

To be successful in this role, you will bring strong clinical assessment, communication, and problem-solving skills along with a passion for patient advocacy. You'll thrive in a collaborative environment where your ability to build relationships, navigate community resources, and support patients through challenging situations makes a meaningful difference every day.

Job Requirements

• Certified Social Worker (CSW) in Nebraska
• Minimum of a bachelor's degree in Social Work (BSW), with the intention of entering into a master's degree program in Social Work (MSW)
• Basic Life Support (BLS) from the American Heart Association

Pay Range

$24.27 - $36.10 /hour
