Job summary

Omaha Home Health Care, part of the Vetter Senior Living family, seeks a Home Health Social Worker to support patients and families at home with education, advocacy, and compassionate care.

You will collaborate with an interdisciplinary team to ensure patients receive the highest level of support, while maintaining dignity and independence in daily living.

Qualifications

• MSW required or preferred
• Strong integrity and patient-centered approach
• Ability to build positive relationships with patients, families, and team
• Excellent communication and problem-solving skills
• Experience in home health preferred, but not required
