Job summary

Outfield Healthcare Partners is seeking a licensed social worker to identify psychosocial, mental and emotional needs and help residents access services in a long-term care setting.

You will work with an interdisciplinary team to complete MDS-informed assessments, support residents’ self-determination, and assist with discharge planning and family education. A compassionate, ethics-driven approach is essential.

Qualifications

• Certified, licensed, or registered in the state of practice, required.
• Bachelor’s Degree in Social Work or a Bachelor’s Degree in Human Services field from an accredited school of social work, required.
• One year of supervised social work experience in a healthcare setting working directly with geriatric individuals.
• Minimum of 2 years of nursing experience in a Skilled Nursing Facility preferred.

Responsibilities

• Completes social history and psychosocial assessment for each resident identifying social, emotional, and psychological needs; uses MDS to guide the care plan.
• Maintains accurate and timely documentation meeting federal/state regulations and center practices.
• Supports residents' right to self-determination and informs about therapeutic alternatives to refused services.
• Develops an interdisciplinary plan of care outlining psychosocial needs and interventions.
• Provides therapeutic support to aid adjustment to the facility and educate residents/families about placement and services.
• Coordinates discharge planning and referrals for home care services prior to community return.
• Collaborates with the interdisciplinary team to protect resident rights and promote psychological well-being.

Education

Bachelor’s Degree in Social Work or a Bachelor’s Degree in Human Services field
