Job summary

Outfield Healthcare Partners seeks an experienced Social Services Director (Licensed Social Worker) to lead social work services in a skilled nursing facility in Indianapolis, IN. This role focuses on psychosocial assessments, care planning, discharge planning, and resident advocacy to support emotional and social well-being.

You will work closely with nursing, rehabilitation, activities, dietary, and administration to ensure resident-centered care, in compliance with CMS regulations and state

Qualifications

• Current licensed, certified, or registered Social Worker credential in the state of practice required.
• Bachelor’s Degree in Social Work (BSW) or related Human Services field from an accredited college or university.
• Minimum of one year of supervised social work experience in a healthcare setting with geriatric populations preferred.
• Experience in SNF/long-term care environments preferred.
• Knowledge of MDS, resident assessments, care planning, discharge planning, PASARR and CMS/state survey requirements.

Responsibilities

• Complete social histories, psychosocial assessments, and resident evaluations.
• Participate in the MDS process and guide individualized care planning.
• Develop, implement, and monitor social service care plans with measurable outcomes.
• Maintain compliant documentation: assessments, progress notes, care plans, PASARR, discharge records.
• Provide emotional support and counseling to residents and families.
• Advocate for resident rights and self-determination in decision-making.
• Coordinate discharge planning with home care services and outside agencies.
• Collaborate with interdisciplinary team to address psychosocial needs and quality of life.

Education
