Qualifications

• Virginia medical license required.
• DEA registration required.
• Board certification in Internal/Family Medicine, Geriatrics, or related field.
• 2+ years of relevant clinical practice.

Responsibilities

• Provide high-quality complex geriatric care in skilled nursing facilities.
• Engage with facility leadership, other providers, patients and families.
• Supervise nurse practitioners and collaborate with the care team.
• Participate in on-call schedule and attend trainings/meetings as required.
• Use mobile-based EHR to document and coordinate care.
