Salem Transitional Care in Oregon is seeking a compassionate Speech-Language Pathologist to support residents in a skilled nursing setting, addressing speech, cognition, and swallowing needs.

You will work with a supportive team to develop personalized treatment plans, track progress, educate families, and ensure timely documentation while helping residents regain independence and quality of life. Qualifications

• Master’s degree in Speech-Language Pathology required.
• Current Oregon Speech-Language Pathology license required.
• Valid CPR certification required.
• Experience in a skilled nursing, long-term care, or rehabilitation setting preferred.
• Experience evaluating and treating speech, language, cognitive-communication, and swallowing disorders preferred.

Responsibilities

• Evaluate residents and create personalized treatment plans.
• Provide hands-on therapy focused on communication, cognition, and swallowing.
• Track progress and adjust care plans to support meaningful outcomes.
• Educate residents, families, and caregivers on exercises and strategies.
• Complete documentation in a timely and accurate manner.
• Collaborate closely with nursing and therapy teams to provide well-rounded care.
