Job summary

Salem Transitional Care is seeking a compassionate Speech-Language Pathologist (SLP) to join our team in a skilled nursing setting. You will work with residents on speech, cognition, and swallowing to support independence and daily communication.

You will evaluate residents, create treatment plans, collaborate with nursing and therapy staff, educate families, and document progress. This role emphasizes patient-centered care within a supportive, team-oriented environment.

Qualifications

  • A licensed Speech-Language Pathologist who is patient-focused, positive, and team-oriented.
  • Strong communication skills and clinical judgment.
  • Experience or interest in geriatric populations and collaboration in care teams.

Responsibilities

  • Evaluate residents and create personalized treatment plans.
  • Provide hands-on therapy focused on communication, cognition, and swallowing.
  • Track progress and adjust care plans to support meaningful outcomes.

Requirements
Education
  • Master’s degree in Speech-Language Pathology required
Licenses/Certification
  • Current Speech-Language Pathology license in the state of Oregon required
  • Valid CPR certification required
Experience
  • Experience in a skilled nursing, long-term care, or rehabilitation setting preferred
  • Experience evaluating and treating speech, language, cognitive-communication, and swallowing disorders preferred
  • Experience working with geriatric populations highly valued
