Job summary

Mayo Clinic is seeking an RN in Senior Services to coordinate nursing care and provide patient/family education across transitions of care. The role involves assessments and education for geriatric patients in their homes or nearby facilities within 20 miles of Rochester, MN.

The ideal candidate has an ACEN/CCNE/NLN CNEA-accredited nursing degree, at least one year of RN experience, and strong communication skills.

Qualifications

• RN must have a baccalaureate nursing degree and be licensed.
• One year of RN experience in applicable care setting is required.
• Excellent communication skills (verbal and written) and teamwork.
• Experience with electronic medical records (EMR) preferred.
• Ability to work flexible hours including days, evenings, nights, holidays and weekends.

Responsibilities

• Coordinate nursing care and patient/family education across transitions of care.
• Provide assessments and education to patients in their homes or assisted living facilities within 20 miles of Rochester.
• Lead or participate in professional practice activities and committees as needed.
• Possibly drive personal vehicle with mileage reimbursement where applicable.
