Job summary

St. Paul’s Senior Services in San Diego is seeking an experienced RN Case Manager to coordinate comprehensive participant care within the PACE program.

Under the supervision of the Clinical Services Manager, you will ensure timely assessments, medication management, and care planning in line with CMS guidelines. You will collaborate with an interdisciplinary team, conduct home visits as needed, maintain medical records, and communicate changes to Care Kinesis and families.

Qualifications

• Current California registered nurse license and CPR certification.
• Minimum one year of experience with elderly or CMS-based programs.
• Strong care coordination and documentation skills.

Responsibilities

• Administer medications per nursing standards and document.
• Coordinate comprehensive participant care with interdisciplinary team.
• Conduct CMS-aligned assessments and develop care plans.
• Travel to participant locations as required.
• Maintain medical records and CMS/NIA charting.
