AltaMed Health Services Corp. is seeking a Registered Nurse Case Manager & Care Coordination Team Lead to oversee all PACE participants in the ambulatory setting.

The role demands strong leadership of a coordinated care team and close collaboration with physicians and social work. The ideal candidate holds an active California RN license with BLS, and has 1+ year in geriatric care and managed care environments, either ambulatory or inpatient.

Qualifications

• Current valid RN license with the California BRN.
• At least 1 year geriatric nursing experience preferred.
• At least 1 year of managed care/case management experience in ambulatory or inpatient setting preferred.
• Valid BLS certification or higher per AHA/Red Cross guidelines.

Responsabilités

• Oversee care coordination for PACE participants in the ambulatory setting.
• Lead RN care coordination team and ensure best practices.
• Collaborate with multidisciplinary teams to optimize patient outcomes.
