Job summary

NHC HealthCare Greenwood is seeking a Registered Nurse to serve as Nurse Assessment Coordinator in Greenwood, SC. The role focuses on coordinating MDS-related processes and ensuring accurate medical records to guide Care Area Triggers and Patient Care Plans.

Candidates should hold an active RN license, preferably CMAC, with geriatric nursing experience and knowledge of SNF Part A Medicare rules. We offer a competitive package and room for advancement.

Qualifications

• Registered Nurse licensed to practice in the state.
• Experience in geriatrics nursing preferred.
• Knowledge of Resident Assessment Instrument (MDS) and rules.
• Understanding of SNF Part A Medicare and consolidated billing.

Responsibilities

• Monitor the medical record to validate MDS coding support.
• Work with interdisciplinary team to ensure timely care plans.
• Support accuracy of documentation through ongoing in-services.
• Coordinate activities to meet deadlines and maintain schedules.
