NHC HealthCare Charleston is seeking an RN, Assessment Coordinator to oversee patient care planning and MDS coding within a geriatrics-focused environment at our Charleston facility. You will collaborate with an interdisciplinary team to ensure accurate documentation and timely care plans.

Ideal candidates hold an active RN license, prefer CMAC certification, and have knowledge of RAI rules and Medicare Part A billing. This on-site role provides competitive pay and benefits.

Qualifications

• Registered Nurse licensed in the state with active RN credential.
• CMAC certification preferred; relevant to assessment coordination.
• Professional experience in geriatrics nursing and patient care.
• Knowledge of Resident Assessment Instrument (RAI) rules and processes.
• Knowledge of SNF Part A Medicare and Consolidated billing procedures.

Responsabilités

• Monitor medical records to validate MDS coding accuracy.
• Collaborate with interdisciplinary team to ensure timely completion of Care Area Triggers and Patient Care Plans.
• Support documentation accuracy through ongoing center partner in-services.
• Demonstrate strong organizational skills to schedule and meet deadlines.
