NHC HealthCare Charleston in Charleston, SC is seeking an RN Assessment Coordinator to oversee MDS coding support and care planning within a geriatrics-focused team.

You will coordinate with a multidisciplinary staff, ensure timely documentation, and support quality patient care while maintaining regulatory compliance and an organized medical records system.

Qualificações

• Registered Nurse licensed in the state.
• CMAC Certification preferred.
• Experience in geriatrics nursing.
• Knowledge of the Resident Assessment Instrument (RAI) including rules and regulations.
• Knowledge of SNF Part A Medicare and Consolidated billing rules.

Responsabilidades

• Schedule and coordinate patient care activities to meet deadlines.
• Monitor medical records to validate MDS coding accuracy.
• Collaborate with the interdisciplinary team to finalize Care Area Triggers and care plans.
• Support documentation quality through ongoing in-services to center partners.

Qualifications

• Registered Nurse - graduate of an accredited school of nursing and currently licensed to practice nursing in the state.
• CMAC Certification preferred.
• Professional experience in the practice of geriatrics nursing.
• Knowledge and expertise in the completion of the Resident Assessment Instrument including all applicable rules and regulations.
• Knowledge of SNF Part A Medicare and Consolidated billing rules.
