Job summary

Water’s Edge Rehab and Nursing Center at Port Jefferson is seeking an Experienced Physical Therapist (PT) for our facility. The PT will assess referred patients, develop and administer treatment programs, and provide direct patient care under physician guidance.

The role requires NY licensure, graduation from an accredited PT program, and supervision of PTAs. Experience in long-term/geriatric care is preferred; strong documentation and collaboration skills are essential.

Qualifications

• Graduated from an accredited Physical Therapy college or university.
• Valid professional license and NY state registration.
• Experience supervising Physical Therapy Assistants.
• Experience in long-term/geriatric care preferred.
• Strong documentation writing skills.
• Experience with evaluations, progress notes, recertifications, discharge docs.

Responsibilities

• Assess referred patients requiring rehabilitation and provide direct patient care.
• Establish and administer treatment programs with goals per patient capacity.
• Document patient condition and educate patients and families in physical therapy methods.
• Collaborate with all disciplines to provide patient-centered care.
• Report to work on time and maintain professional standards.
• Perform duties assigned by the Director of Rehabilitation.
