Job summary

Water’s Edge Rehab and Nursing Center at Port Jefferson is seeking an Experienced Physical Therapist (PT) for our facility. The PT will assess referred patients, develop and implement treatment plans, and document patient progress while educating patients and families.

The role requires an accredited PT degree and a valid license, with supervision of PT Assistants and collaboration across disciplines. Prior long-term care or geriatric experience is preferred; excellent documentation is essential.

Qualifications

• Graduated from an accredited Physical Therapy college or university.
• Licensed to practice Physical Therapy in the state of practice.
• Experience supervising Physical Therapy Assistants.
• Experience in long-term care/geriatric care preferred.
• Strong documentation writing skills.

Responsibilities

• Assess referred patients for rehabilitation needs and provide direct patient care.
• Establish and administer treatment programs with goals based on patient capacity.
• Document patient condition and educate patients and families.
• Supervise Physical Therapy Assistants and collaborate with all disciplines.
• Adhere to scheduled hours and maintain professionalism.
