Job summary

Water’s Edge Rehab and Nursing Center at Port Jefferson is seeking an Experienced Physical Therapist (PT) for our facility. The PT will assess referred patients needing rehabilitation, provide direct care, and develop treatment plans under physician direction.

Responsibilities include documenting patient progress, educating patients and families, and supervising Physical Therapy Assistants. Long-term care experience is preferred; punctuality and professionalism are essential.

Qualifications

• Must have graduated from an accredited Physical Therapy college or university.
• Must have a valid professional license and registration for state of practice.
• The Physical Therapist is responsible for overseeing and supervising Physical Therapy Assistants.
• Experience with long-term care and/or geriatric care is preferred but not required.
• Well versed with documentation writing.
• Evaluations, weekly progress notes, recertification and discharge documentations.
• Collaborates with all disciplines to provide best patient-centered treatment and care.
• Exceptional patient service skills and ability to demonstrate clinical excellence.

Responsibilities

• Assess referred patients requiring rehabilitation and provide direct patient care.
• Establish and administer a treatment program with goals according to patient capacity.
• Document and record patient condition and educate patients and families in PT methods.
• Report to work on time and adhere to scheduled hours with professionalism.
• Perform all other duties as assigned by Director of Rehabilitation.
