Job summary

Water’s Edge Rehab and Nursing Center at Port Jefferson is seeking an Experienced Physical Therapist (PT) for our facility. The PT will assess referred patients, develop and administer treatment plans, and educate patients and families in therapy methods.

The role requires graduation from an accredited PT program, active licensure, and supervision of PT assistants. Geriatric/long-term care experience is preferred, with strong documentation and collaborative care.

Qualifications

• Must be a graduate of an accredited Physical Therapy college or university.
• Must hold a valid professional license and registration for the state of practice.
• Oversee and supervise Physical Therapy Assistants.
• Experience with long-term care and/or geriatric care preferred.
• Strong documentation writing skills.
• Perform evaluations, weekly progress notes, recertification and discharge documents.
• Collaborate with all disciplines to provide patient-centered care.
• Excellent patient service and clinical excellence.
• Report on time and maintain professionalism.
• Perform duties as directed by the Director of Rehabilitation.
• Maintain confidentiality, HIPAA compliance, and Residents' rights.

Responsibilities

• Assess referred patients requiring rehabilitation and provide direct patient care.
• Establish and administer a treatment program with goals based on patient capacity and tolerance.
• Document patient condition and educate patients and families in therapy methods.
