Job summary

Water’s Edge Rehab and Nursing Center at Port Jefferson is seeking an Experienced Physical Therapist (PT) to join our facility. The role involves assessing referred patients, designing treatment plans, and delivering direct patient care.

Candidates must have graduated from an accredited PT program, hold a valid license, and be able to supervise PT assistants. Prior LTC/geriatric experience and strong documentation skills are preferred.

Qualifications

• Graduate from an accredited Physical Therapy college or university.
• Valid professional license and registration for state of practice.
• Experience supervising Physical Therapy Assistants.
• Experience with long-term care or geriatric care preferred.
• Strong documentation skills and maintenance of records.
• Ability to collaborate with all disciplines to provide patient-centered care.
• Compliance with confidentiality and HIPAA.

Responsibilities

• Assess referred patients and provide direct patient care.
• Establish and administer treatment programs with goals based on patient capacity.
• Document patient condition and educate patients and families.
• Perform evaluations, weekly progress notes, recertification and discharge documentations.
• Collaborate with all disciplines to provide best patient-centered treatment and care.
• Report to work on time, adhere to scheduled hours and demonstrate professionalism.
