Water’s Edge Rehab and Nursing Center at Port Jefferson is seeking an experienced Physical Therapist to join our facility. You will assess referred patients, develop and administer treatment plans, and set goals in collaboration with the physician.

Qualifications

• Graduated from an accredited Physical Therapy college or university.
• Valid state license and registration.
• Oversee and supervise Physical Therapy Assistants.
• Long-term care or geriatric care experience preferred.
• Strong documentation and patient progress notes.

Responsibilities

• Assess referred patients requiring rehabilitation and determine care plan.
• Develop and administer treatment programs with goals based on patient capacity.
• Document patient conditions and educate patients and families.
• Collaborate with disciplines to provide patient-centered care.
• Oversee Physical Therapy Assistants and ensure clinical excellence.
• Maintain confidentiality and HIPAA compliance.
