Job summary

Elderwood is seeking a compassionate Occupational Therapist to join our skilled nursing and rehabilitation team at North Creek. You will evaluate and treat residents recovering from illness, injury, surgery, and other functional decline, focusing on ADLs, mobility, safety, cognition, and independence.

This role emphasizes collaboration with an interdisciplinary team and documentation, with opportunities to mentor others and contribute to care planning, resident education, and meaningful daily

Qualifications

• Graduation from an approved school of occupational therapy.
• State license to practice occupational therapy.
• Two years’ experience in occupational therapy preferred.
• Ability to communicate well verbally and in writing.
• Current CPR/BLS credential.

Responsibilities

• Provide skilled occupational therapy evaluations and treatments in accordance with physician orders.
• Develop and update individualized treatment plans and care goals.
• Maintain timely and accurate clinical documentation.
• Collaborate with interdisciplinary teams regarding resident progress and discharge planning.
• Participate in resident care planning meetings.
• Support resident and family education initiatives.
• Mentor and provide clinical support as needed.
