Qualifications

• Graduate of an accredited OT program with an active OT license.
• Adherence to Code of Ethical Conduct and state/federal regulations.

Responsibilities

• Evaluate patients and create individualized treatment plans.
• Collaborate with the interdisciplinary team to achieve patient goals.
• Provide flexible scheduling for patient care and supervise COTAs in SNF settings.
