Job summary

Elderwood at Lockport is seeking a compassionate Occupational Therapist to join our skilled nursing and rehabilitation team. You will evaluate residents, treat them, and develop ADL-focused programs to improve function.

Work closely with PT/SLP/Nursing to achieve rehab goals, document care plans, and help residents regain independence. This Regular Full-Time OT role offers flexibility, strong teamwork, and meaningful daily impact.

Qualifications

• Graduation from an approved school of occupational therapy is required.
• State license to practice occupational therapy is required.
• Two years' experience in occupational therapy is preferred.
• Ability to communicate well verbally and in writing.
• Current, valid CPR/BLS credential.

Responsibilities

• Provide skilled occupational therapy evaluations and treatments in accordance with physician orders.
• Develop and update individualized treatment plans and care goals.
• Maintain timely and accurate clinical documentation.
• Collaborate with interdisciplinary teams regarding resident progress and discharge planning.
• Participate in resident care planning meetings.
