The role requires an active RN license in the state, CPR certification, and 1+ years in a skilled/sub-acute or memory care setting. Strong teamwork and communication skills are essential.

Qualifications

• Associate’s degree in nursing or equivalent required.
• Area of study: Nursing.
• 1+ years of experience in skilled, sub-acute, or memory care setting.
• Active RN license in the state of practice with CPR certification.
• English fluency for speaking, reading, writing, and communication with staff and residents.
• Basic computer skills for documentation and coordinate care.

Responsibilities

• Supervise/ administer medications per Federal & State regulations and facility guidelines.
• May oversee admissions including assessments and documentation.
• Develop and maintain acute and long-term care/service plans.
• Track clinical data (e.g., wounds, infection control) under supervision.
• Manage discharges, assessments, and clinical documentation as needed.
• Provide health-related consultations to residents within the community.
• Notify physicians/family of health changes and document appropriately.
• Respond promptly to emergencies and coordinate with staff across departments.
• Maintain confidentiality of resident information and participate in Safety/Disaster Plan.
• Provide educational sessions for residents and staff.
