Job summary

Habitat Health, expanding its PACE-based care model in California, seeks a Recreational Therapist (CTRS) to support the physical, cognitive, emotional, and social well‑being of older adults. You will work as part of the interdisciplinary team to assess, plan, and implement therapeutic recreation interventions tailored to individual goals within compliant regulatory frameworks.

Responsibilities include conducting assessments, developing SMART goals, coordinating with clinical and social care

Qualifications

• Must be a Certified Therapeutic Recreation Specialist (CTRS).
• Bachelor’s degree in Recreational Therapy or related field or equivalent work experience.
• 1–3 years of experience providing recreational therapy to aging and frail populations.
• Ability to independently plan, implement, and evaluate therapeutic recreation interventions.
• Experience conducting functional assessments and documenting progress.
• Strong communication and collaboration skills with IDT.

Responsibilities

• Conduct therapeutic recreation assessments and document findings per PACE requirements.
• Develop and implement individualized and group interventions.
• Contribute to IDT care planning with SMART goals.
• Adapt activities to diverse functional levels and cultures.
• Monitor progress and update care plans.
• Maintain safe, organized activity spaces and supplies.
• Engage participants with cognitive impairment using suitable approaches.
• Identify opportunities to improve programming and engagement.
