• Develop and implement individual and group therapy interventions.
• Contribute to care planning with SMART goals and intervention plans.
• Adapt activities for diverse functional levels and cultural preferences.
• Monitor progress and update care plans; ensure safety in day centers and community settings.
• Support care transitions to home, SNF, or hospital.
