Job summary

Outcome HC is seeking a Full-Time Physical Therapist Assistant to join our rehabilitation team at the Country Arch Care Center in New Jersey. The role focuses on helping seniors regain mobility, independence, and quality of life in a skilled nursing facility.

You will implement treatment plans under a licensed Physical Therapist, document progress in EMR, participate in care plan meetings, and work with an interdisciplinary team.

Qualifications

• Active Physical Therapist Assistant license in New Jersey (or eligible to obtain).
• Mentorship and training provided for new grads.
• Experience in SNF/geriatrics a plus.

Responsibilities

• Implement treatment plans to improve strength, balance, gait, and functional mobility.
• Treat residents under supervision of a licensed Physical Therapist.
• Document progress in EMR and participate in care plan meetings.
