Job summary

Outcome HC at Harbour View Senior Living in New Jersey seeks a Full-Time Physical Therapist Assistant to join the on-site rehabilitation team. The PTA will implement therapy plans to improve mobility and independence under a licensed PT, collaborating with physicians, nurses, and other therapists.

New grads are welcome, with mentorship available; NJ licensure is required or eligible, and prior SNF/geriatrics experience is a plus. The role uses EMR and emphasizes compassionate patient care.

Qualifications

• Active Physical Therapist Assistant license in New Jersey is required or eligible.
• New grads are welcome and will receive mentorship and training.
• Experience in SNF, long-term care, or geriatrics is a plus.

Responsibilities

• Implement treatment plans developed by the Physical Therapist to improve residents' strength, balance, gait, and functional mobility.
• Treat residents with orthopedic, neurological, and post-surgical conditions under supervision.
• Collaborate with physicians, nurses, occupational and speech therapists.
• Document patient progress accurately in the electronic medical records system.
• Participate in care plan meetings and family conferences.
• Provide compassionate, patient-centered care in a team environment.
• Report to the Physical Therapist and Director of Rehabilitation.
