Job summary

Outcome HC in New Jersey is seeking a Full-Time Physical Therapist Assistant to join our rehabilitation team at Country Arch Care Center. You will implement treatment plans under supervision to improve residents' mobility and independence. We welcome new grads and provide mentorship.

Prior SNF, long-term care, or geriatric experience is a plus. You will collaborate with physicians, nurses, and therapists, document progress in EMR, and participate in care planning.

Qualifications

• Active NJ PTA license is required or eligible.
• New grads welcome; mentorship and training provided.
• Experience in SNF/geriatrics is a plus.

Responsibilities

• Implement treatment plans under Physical Therapist supervision to improve mobility, balance, gait and strength.
• Treat residents with orthopedic, neurological, and post-surgical conditions.
• Collaborate with physicians, nurses, OT and SLP teams.
• Document patient progress accurately in EMR.
• Participate in care plan meetings and family conferences.
• Provide compassionate, patient-centered care in a supportive team environment.
