Job summary
Los Gatos, California-based Skilled Nursing Facility seeks a Physical Therapist to evaluate patients, design individualized plans, and guide recovery for geriatric and post-acute populations to improve mobility and independence.
You will supervise PT Assistants, monitor progress, educate patients and caregivers, and ensure compliant documentation. California PT license and BLS are required; 1 year SNF/geriatric experience preferred.
Qualifications
- Active state license as a Physical Therapist in California.
- Current BLS/CPR certification.
- Minimum of 1 year of PT experience, preferably in SNF or geriatric setting.
- Strong clinical reasoning and leadership skills.
- Ability to work collaboratively within an interdisciplinary team.
Responsibilities
- Evaluate, diagnose, and treat patients with physical impairments in a Skilled Nursing Facility setting.
- Develop and implement individualized treatment plans based on patient needs and clinical findings.
- Supervise and collaborate with Physical Therapist Assistants to ensure effective care delivery.
- Monitor patient progress and modify treatment plans as needed.
- Educate patients and caregivers on mobility, safety, and home exercise programs.
- Maintain accurate and timely documentation in compliance with facility and regulatory standards.
