Job summary

A Skilled Nursing Facility in San Rafael, California is seeking a dedicated Physical Therapist to evaluate and treat geriatric patients. You will create individualized plans and guide patients toward greater independence within an interdisciplinary team.

Responsibilities include supervising assistants, monitoring progress, educating patients and caregivers, and maintaining compliant documentation. A CA license and BLS/CPR are required.

Qualifications

• Active state license as a Physical Therapist in California.
• Current BLS/CPR certification.
• Minimum of 1 year of PT experience, preferably SNF/geriatric.

Responsibilities

• Evaluate, diagnose, and treat patients in a SNF setting.
• Develop and implement individualized treatment plans.
• Supervise and collaborate with PT Assistants to ensure care quality.
• Monitor progress and modify plans as needed.
• Educate patients and caregivers on mobility, safety, and home exercise.
