Job summary

Storybrook Care & Rehabilitation in Northern Colorado is seeking a compassionate Physical Therapist to join our in-house therapy team at a midsize geriatric facility. You will develop individualized treatment plans, guide patients toward improved mobility, and collaborate with a seasoned clinical team.

The role offers full-time, part-time, or PRN arrangements, with opportunities to supervise therapy staff. A current state license and commitment to evidence-based practice are required.

Qualifications

• Active State Licensure in good standing.

Responsibilities

• Perform treatments and exercises to help patients recover from illnesses and injuries.
• Develop personalized treatment plans and use evidence-based techniques to improve mobility.
• Teach patients exercises and therapeutic massage to ease pain and promote healing.
• Supervise and collaborate with PT assistants and rehabilitation aides as needed.
• Document all progress, goals, and regulatory-required information in patient records.
