Job summary

Storybrook Care and Rehab in Northern Colorado is seeking a compassionate Occupational Therapist to join our in-house therapy team in a midsize facility with low staffing ratios.

You will screen and treat patients with cognitive and ADL impairments, develop individualized plans, document outcomes, supervise aides and PTAs, and collaborate with the Director of Rehabilitation to ensure high-quality, compliant care for our geriatric residents.

Qualifications

• Must hold active State Licensure as an Occupational Therapist.
• Experience in geriatric rehabilitation preferred.
• Ability to document outcomes and care plans per regulations.
• Supervise PTAs and rehab aides as needed.

Responsibilities

• Screen and evaluate patients with cognitive and/or ADL deficits; develop care plans.
• Utilize standardized assessments and evidence-based practice.
• Provide rehabilitative, skilled treatment per regulatory standards.
• Document treatment minutes and progress in electronic records.
• Communicate progress and plans with supervisor and care team.
• Prepare discharge summaries in compliance with policy.
• Supervise PTAs, aides and/or students as required.
