Therapy Management Corporation (TMC) is seeking a Physical Therapist to empower adults and geriatrics through compassionate, patient-centered therapy. You will assess, treat, and document outcomes in collaboration with interdisciplinary teams to improve independence and quality of life.

Qualifications

• Licensed Physical Therapist (PT) or license eligible in the state of practice.
• Graduate of an accredited Physical Therapist Program.
• Interest or experience in skilled nursing, long-term care or geriatric rehabilitation.
• Passion for patient-centered care and improving quality of life for older adults.

Responsibilities

• Evaluate patients and provide personalized physical therapy treatments to improve mobility and independence.
• Develop and implement patient-centered treatment plans for long-term goals.
• Deliver hands-on therapy interventions to support recovery in older adults.
• Help patients restore functional mobility and safely perform daily activities.
• Collaborate with interdisciplinary teams, caregivers and nursing staff to drive outcomes and continuity of care.
• Track patient progress and maintain accurate clinical documentation.
