Qualifications

• Licensed Physical Therapist in the practicing state or license eligible.
• Graduate of an accredited Physical Therapist Program.
• Interest or experience in skilled nursing or geriatric rehabilitation.
• Passion for patient-centered care and improving quality of life for older adults.

Responsibilities

• Evaluate patients and provide personalized physical therapy treatments to improve mobility and independence.
• Develop and implement patient-centered treatment plans focused on long-term goals.
• Deliver hands-on therapy interventions to support recovery and functional outcomes in older adults.
• Help patients restore mobility and safely perform daily activities with confidence.
• Partner with interdisciplinary teams to drive meaningful outcomes and continuity of care.
• Track progress, maintain accurate documentation, and adapt plans as needed.
