Job summary

Eventus WholeHealth seeks a licensed clinical social worker or related therapist to provide recovery-oriented psychotherapy in skilled nursing and assisted living settings. You will work as part of a multidisciplinary team addressing anxiety, depression, and other mental health concerns in residents, delivering individualized treatment plans.

Responsibilities include assessment, coordination with physicians and facility staff, documentation, and adherence to HIPAA and regulatory guidelines.

Qualifications

• Master’s degree in social work or related field is required.
• Current state license to practice (LCSW/LISW/LPCC/LMFT/LCMHC) required.
• Experience delivering psychotherapy and recovery-oriented rehabilitation in long-term care is preferred.

Responsibilities

• Assess residents with consent to determine needs and develop treatment plans.
• Coordinate with primary care physicians, facility staff, and Eventus support staff.
• Document treatment plans and sessions per policy and provide timely records for medical charts.
• Maintain professional communications with facility staff and adhere to HIPAA and regulatory guidelines.
• Consult with clinical supervisor for challenging cases or requests.
