Job summary

Eventus WholeHealth seeks a licensed therapist to provide recovery‑oriented rehabilitation services in group or individual settings within skilled nursing and assisted living facilities. You will work as part of a multidisciplinary team delivering care to residents with anxiety, depression and other mental health needs.

Responsibilities include assessment, treatment planning, documentation, and collaboration with facility staff while maintaining HIPAA and regulatory compliance.

Qualifications

• Master’s in social work and current state license.
• Licensed as LCSW/LISW/LPCC/LCMHC/LMFT in the practicing state.
• Experience in long-term care settings preferred.

Responsibilities

• Assess residents with consent to determine needs and develop treatment plans.
• Consult with primary care physicians, facility staff and support staff as needed.
• Document treatment plans and sessions in EMR within policy timelines.
• Coordinate with facility staff on care needs and communicate findings.
• Maintain confidentiality and HIPAA compliance at all times.
• Adhere to regulatory standards and professional ethics.
