Eventus WholeHealth in Statesville, NC seeks a licensed clinical social worker or related licensed professional to provide recovery-oriented rehabilitation services in long-term care settings.

You will work within a multidisciplinary team, developing treatment plans, addressing anxiety, depression and other needs, and ensuring care aligns with HIPAA and regulatory standards. Strong communication and documentation skills are essential.

Qualifications

• Licensed to practice in the state for the applicable track (psychologist or social worker).
• Master’s in social work for therapists or doctoral degree for psychologists is required.
• Experience in long-term care settings preferred.

Responsabilités

• Assess each resident and develop treatment plans meeting needs.
• Coordinate with primary care, family, facility staff and Eventus support.
• Document plans and sessions; provide written records within 48 hours.
• Maintain communication with facility staff and residents’ care teams.
• Adhere to Eventus WholeHealth policies, HIPAA, and regulatory guidelines.
