Job summary

Eventus WholeHealth seeks licensed clinicians to provide recovery-oriented rehabilitation services for residents in skilled nursing and assisted living facilities. Clinicians work in group or individual settings as part of a multidisciplinary team.

Qualifications

• Master’s degree in social work with licensure to practice in the state.
• Licensed Social Worker licenses (LCSW/LISW/LPCC/LCMHC/LMFT) to practice.
• Strong knowledge of assessment, psychotherapy, DSM, and ethical guidelines.
• Ability to follow treatment plans and ensure medical necessity.

Responsibilities

• Assess residents with consent and develop individualized treatment plans.
• Document treatment plans and psychotherapy sessions with timely updates.
• Consult with residents' primary care physicians, families, facility staff and support teams.
• Maintain confidentiality and HIPAA compliance; conduct professional practice.
