Job summary

Eventus WholeHealth seeks licensed clinicians to join our team serving adults in skilled nursing and assisted living facilities. You will provide recovery-oriented rehabilitation in group or individual settings within a multidisciplinary team, addressing anxiety, depression, and other mental health needs.

You will collaborate with primary care and facility staff, adhere to HIPAA and regulatory guidelines, and document treatment plans and progress in the EMR.

Qualifications

• MSW degree and current state licensure required.
• Doctoral-level psychologists require doctoral license.
• Active state license required for therapists (LCSW/LISW/LPCC/LCMHC/LMFT).

Responsibilities

• Assess residents with consent to determine needs and develop treatment plans.
• Consult with primary care physician, family, facility staff as needed.
• Document treatment plans and psychotherapy sessions for medical records within policy.
• Meet with care coordinators to review facility care needs and communicate findings.
• Maintain open communications with facility staff and provide care plan guidance.
• Follow Eventus WholeHealth policies, HIPAA, and regulatory guidelines.
• Maintain professional conduct and patient confidentiality at all times.
• Consult with supervisor when unfamiliar requests arise.
