Job summary

Telos Health Systems is seeking a Psychotherapist (LCSW, LPC, LMFT, PsyD, or PhD) to deliver mental health evaluations and psychotherapy to residents in Skilled Nursing and Assisted Living Facilities, Aliquippa, PA. The role offers clinical autonomy, strong support, and flexible scheduling.

Qualifications

• Master's degree in a relevant mental health field.
• Doctoral degree in psychology (PsyD/PhD) in psychology.
• Ability to work independently while collaborating with a clinical team.
• Strong communication and clinical documentation skills.
• Preferred: experience with geriatric populations.
• Preferred: experience in Skilled Nursing, Assisted Living, or long-term care settings.

Responsibilities

• Conduct comprehensive mental health assessments and diagnostic evaluations.
• Provide individual psychotherapy and ongoing treatment services.
• Develop, implement, and update individualized treatment plans with the multidisciplinary care team.
• Maintain accurate, timely, and compliant clinical documentation.
• Coordinate care with facility staff, medical providers, and other healthcare professionals.
• Adhere to all federal, state, and organizational standards, policies, and regulations.
