Qualifications

• Graduation from an accredited Nurse Practitioner program and ANCC certification as a PMHNP.
• Active DEA license.
• BLS and ACLS certification.
• Current Nurse Practitioner license in the state of practice.

Responsibilities

• Psychiatric Care and Rounding: Conduct patient rounds and manage psychiatric care within SNF, LTC, AL, and IL settings.
• Collaborative Care Coordination: Partner with physicians, consultants, referral sources, and facility staff for coordinated care.
• Patient Evaluation and Management: Assess mental health needs, perform exams, and gather medical histories.
• Diagnostics and Therapeutics: Order tests, diagnose, and develop care plans.
• Medication and Treatment Management: Prescribe medications and provide injections as needed.
• Patient and Family Education: Educate patients and families on medications and care plans.
• Acute Care Collaboration: Work with teams to address urgent needs.
