Job summary

Curana Health is seeking a Psychiatric Nurse Practitioner (PMHNP) to deliver high-quality geriatric psychiatric care across SNF, LTC, AL, and IL settings. You will join a collaborative team and provide personalized mental health services with a focus on continuity of care in post-acute and long-term care environments.

This role supports flexible part-time schedule (2-3 days/week) and travel to facilities. Active PMHNP license, ANCC certification, and DEA are required.

Qualifications

• Graduation from an accredited Nurse Practitioner program and ANCC PMHNP certification.
• Active DEA license.
• Current Nurse Practitioner license in state of practice.
• BLS and ACLS certification.

Responsibilities

• Psychiatric Care and Rounding: Conduct patient rounds and manage psychiatric care within SNF, LTC, AL, and IL settings.
• Collaborative Care Coordination: Partner with attending physicians, consulting specialists, referral sources, and facility staff to ensure comprehensive, coordinated care.
• Patient Evaluation and Management: Assess and manage the mental health needs of residents, perform physical exams, and gather complete medical histories.
• Diagnostics and Therapeutics: Order and interpret lab and diagnostic tests, make preliminary diagnoses, and develop effective therapeutic care plans for mental and physical health.
• Medication and Treatment Management: Prescribe and administer medications and treatments; provide injections as necessary.
• Patient and Family Education: Educate patients and families on medications, care plans, and preventive mental health practices.
