Job summary

Curana Health is seeking a Psychiatric Nurse Practitioner to deliver high-quality mental health services across SNF, LTC, AL, and IL communities. The role emphasizes compassionate geriatric psychiatry within a multidisciplinary team and ensures continuity of care across post-acute and long-term settings.

Qualifications

• Graduation from an accredited Nurse Practitioner program with PMHNP certification.
• Active DEA license.
• BLS and ACLS certification.
• Current Nurse Practitioner license in the state of practice.
• Willingness to travel to multiple facilities.

Responsibilities

• Conduct patient rounds and manage psychiatric care within SNF, LTC, AL, and IL settings.
• Collaborate with attending physicians, specialists, referral sources, and facility staff to ensure coordinated care.
• Assess and manage residents' mental health needs and medical histories.
• Order and interpret lab and diagnostic tests; develop treatment plans.
• Prescribe and administer medications and treatments; provide injections as necessary.
• Educate patients and families on medications, care plans, and preventive mental health practices.
• Coordinate acute psychiatric and medical care with facility teams.
