Job summary

Eventus WholeHealth seeks an Advanced Practice Clinician (NP/PA) to deliver behavioral health and psychiatric medication management in skilled nursing and senior living communities. You will perform routine rounds, evaluate and manage mental health conditions, and work with interdisciplinary teams to support residents’ emotional and cognitive well-being.

Qualifications

• Current NP or PA license in the state of practice.
• Professional patient care experience.
• Experience in geriatrics, family medicine, or psychiatry preferred.
• Exceptional patient-centered service skills, including communication and listening.
• Strong collaboration across interdisciplinary teams.

Responsibilities

• Provide behavioral health and medication management within skilled nursing and senior living communities.
• Conduct routine patient rounding and ongoing management of mental health conditions.
• Collaborate with interdisciplinary teams to support residents’ emotional and cognitive well-being.
• Coordinate with facility partners to ensure continuity of care and optimize treatment plans.
• Promote a therapeutic environment and value-based care for residents and families.
