Eventus WholeHealth is seeking an Advanced Practice Clinician (NP/PA) to deliver behavioral health and psychiatric care in skilled nursing and senior living communities. You will round on residents, evaluate conditions, and manage medications within a collaborative team to improve cognitive and emotional well‑being.

This role emphasizes value‑based care, proactive treatment plans, and close coordination with facility partners to ensure continuity of care and a therapeutic environment.

Qualifications

• Current NP or PA licensure in the state of practice (Indiana).
• Experience in geriatrics, family medicine, or psychiatry preferred.
• Exceptional communication and teamwork with residents, families, and staff.

Responsibilities

• Provide behavioral health and psychiatric medication management in skilled nursing and senior living communities.
• Conduct routine patient rounding, evaluation, and ongoing management of mental health conditions.
• Collaborate with interdisciplinary teams to support residents’ emotional and cognitive well-being.
