You will perform routine rounds, assess and adjust treatment, and collaborate with interdisciplinary teams to support residents’ emotional and cognitive well‑being, with a value‑based care approach.

Qualifications

• Current NP or PA licensure in the state of practice.
• Previous professional patient care experience.
• Experience in geriatrics, family medicine, or psychiatry preferred.
• Patient-centered care with compassionate communication and active listening.
• Strong collaboration across interdisciplinary teams.

Responsibilities

• Provide behavioral health and psychiatric medication management within skilled nursing and senior living communities.
• Routine patient rounding and ongoing management of mental health conditions.
• Collaborate with interdisciplinary teams to support residents’ emotional and cognitive well‑being.
• Partner with facility staff to ensure continuity of care and optimize treatment plans.
