Jobot seeks a Physician to provide primary care for a senior population in a collaborative, patient-centered environment in Stockton, CA. You will focus on chronic disease management, preventative care, care coordination, and preserving independence.

The role emphasizes a Monday–Friday schedule with no weekends or after-hours calls, offering a competitive compensation package of $250k–$300k per year plus bonus potential and comprehensive benefits.

Qualifications

• Must have geriatrics experience with elderly or frail populations.
• Strong teamwork across nurses, specialists, and clinicians.
• Excellent communication with patients, families, and team members.
• Experience managing chronic diseases and preventive care for seniors.
• Care coordination and care planning across settings.

Responsabilités

• Provide primary medical care and ongoing management for seniors.
• Diagnose and manage acute and chronic conditions in an elderly population.
• Coordinate with nurses, specialists, and other clinicians to deliver care.
• Order and interpret tests and adjust treatment plans as needed.
• Educate patients and families on disease management and prevention.
• Supervise and collaborate with APPs and physician assistants.
• Maintain accurate clinical documentation.
