Job summary

Jobot is seeking a Nurse Practitioner to provide primary care for a senior population in Stockton, CA in a collaborative, patient-centered setting. Focus on chronic disease management, care coordination, and helping older adults maintain health.

This on-site, permanent role offers a weekday schedule, no weekends or after-hours calls, competitive compensation of $140k-$160k, and benefits including medical, dental, vision, life insurance, 401(k), PTO, CME allowance.

Qualifications

• Active California NP and RN licenses.
• Graduate of an accredited NP program.
• Minimum 5 years nursing experience.
• At least 1 year with elderly/frail population.
• Experience in primary care or geriatrics is preferred.
• DEA registration within 6 months of hire; BLS certification.

Responsibilities

• Perform initial, annual, and ongoing patient assessments with care plans.
• Manage acute and chronic conditions for a frail elderly population.
• Collaborate with physicians, nurses, specialists, and the care team.
• Provide patient and family education on treatment plans and disease management.
• Coordinate specialty referrals, hospital transitions, and follow-up care.
• Maintain timely and accurate clinical documentation.
